What allowance for each laying pattern?

5% is enough for a straight lay in a simple room, 10% covers typical cuts, 15% is needed for a diagonal lay or a room with many angles.

Should I include skirting and thresholds?

No: this covers the floor area only. Skirting boards, thresholds and step nosings are counted in linear metres and added separately.
